- Include copies of the national identification cards of your parents, guardians, or spouse.
- If applicable, provide proof of income or a year’s worth of IRP5.
- Include proof of income for your parents, guardians, or spouse.
- SASSA grant recipients do not need to provide proof of income.
- Applicants with disabilities must complete and sign a Disability Annexure A Form, which should be submitted.
- If identified as a Vulnerable Child by the Department of Social Development, you must also complete and sign a Vulnerable Child Declaration and Consent Form.
