A problem-solving approach to nursing interventions is called a nursing care plan, or NCP. These plans are used by registered nurses (RNs) to identify and treat patients’ conditions.

Why do you need a nursing care plan?

You may be implementing nursing care plans without referring to them as such if the medical setting in which you work always has a method for checking off all of the needs listed below. You should have a nursing care plan for the following reasons: NCPs:

• Facilitate effective, standardized, and comprehensive care: A nursing care plan ensures that all medical professionals receive the same patient information, enabling them to provide comprehensive care.
  Ensure that patient-centered care is provided: This makes it easier for nurses and other medical professionals to communicate with one another and provide constant patient assistance. The plan aids the nurse assigned to support the patient even if the patient requires specialized medical attention due to their physical or physiological conditions.
  Maintain impeccable records: The care plan makes it easier for nurses to keep the patient’s medical records clear. The patient’s care is documented by these records.
  Define the roles of nurses and other members of the team in the care process: A well-written care plan ensures that everyone involved in the patient’s care understands their roles and responsibilities.
  Participate in patients’ own care plans: Patients gain a better understanding of the significance of their conditions and the ways in which they can improve their health as a result.
  Analyze the care’s effectiveness: It is essential to conduct regular assessments of the patient’s condition to ensure that they are improving or that the care plan is effective.
  Aid nurses in setting care goals: The nurses are responsible for setting goals for themselves after the diagnosis has been made to ensure that the patient’s condition improves quickly. These objectives may be short-term or long-term.
  Encourage the care team to work together: All team members are able to support one another in this way while providing direct patient care. It is much simpler for the team to take care of the patient because they all have access to the patient’s medical records and history.
  Check for compliance: Because it shows the patient’s improvement graph, this is one of the most important parts of a nursing care plan. The patient’s good care is demonstrated by this documentation.

How to create a nursing care plan

The five components of the nursing process serve as the foundation for nursing care plans. What they are and how they relate to the plan are as follows (when admitting a new patient, nurses should fill out the information on each element):

• Assessment: Examine the condition of a patient as well as any results from diagnostic tests or lab tests. To put it succinctly, this is the patient’s medical history, which is used to determine whether the patient has any food or medication allergies.

Examples of nursing care plans

  Self-esteem: acceptance in the workplace and community, recognition of one’s own accomplishments, a sense of control or empowerment, and acceptance of one’s appearance.
  Self-actualization: Empowerment, development, and the capacity to comprehend the perspectives of others are all aspects of growth.
