The nursing process is a method for providing high-quality nursing care that is patient-centered, systematic, and based on evidence. There are five steps in it: evaluation, diagnosis, assessment, outcomes/planning, implementation, and assessment

Your primary responsibility as a nurse is to ensure that your patients receive safe care according to the medical team’s care plan. You will be responsible for deciding what to do with the information you collect and analyze about patients throughout your shift. You will manage the needs of your patients in a methodical manner if you adhere to the nursing procedure. The nursing process provides a practice framework for the nurse to adhere to in order to ensure that the patient’s needs are met. We describe the nursing process and the steps involved in this universal nursing tool in this article.

The five steps of the nursing process

  • The nursing process is a five-step, evidence-based process that helps nurses think about their patient’s overall picture and care plan holistically. The nursing process is something that nurses learn in nursing school and use sporadically while providing patient care in the workplace.
  • The nurse must use clinical judgment when devising a day-to-day strategy for a shift with patients. The nursing process enables the nurse to collect data, prioritize issues, and establish objectives for each patient. When it comes to making decisions regarding patient care, the nursing process becomes more natural as nurses gain experience.

The steps are as follows:

The Nursing Process (ADPIE)

Assessment Collect information about a patient’s condition and determine their health requirements.
Diagnosis Determine whether the patient is currently experiencing any actual or potential health issues.
Plan Create a nursing care plan that outlines the steps that will be taken to meet the needs that have been identified and achieve the desired outcomes for the patient.
Implementation Implement the care plan and keep an eye on the patients’ development.
Evaluation Determine whether the treatment plan was successful. The procedure is then repeated if necessary until the patient is discharged or reaches all of their health care objectives.
