Each of the approximately 16,000 Medicare or Medicaid-certified nursing homes in the nation receives quality ratings from Nursing Home Compare, which is managed by the Centers for Medicare and Medicaid Services (CMS). The three most important aspects of each facility determine its star rating, which ranges from one star to five stars. results from health inspections, quality indicators, and staffing levels. There is also a rating for the whole thing.
How CMS calculates ratings
The following procedures are used by CMS to determine the Overall Five-Star rating:
- Begin with the rating for health inspections.
Add 1 star if the “Staffing” rating is four or five stars and more noteworthy than the Wellbeing Investigations Rating. If the “Staffing” rating is one star, subtract one star.
If the “Quality Measures” rating is five stars, add one star; If the “Quality Measures” rating is one star, remove one star.
Based on the ratings for “Staffing” and “Quality Measure,” the “Overall” rating cannot be increased by more than one star if the “Health Inspections” rating is one star.
The maximum “Overall” rating for a nursing home that is a Special Focus Facility is three stars.
What are the ratings?
- The rating for health inspections is based on the three most recent comprehensive (annual) inspections as well as complaints-related inspections conducted within the previous three years. Recent inspections receive more attention.
The Quality Measures (QM) rating is created by combining the values of nine QMs, which are a subset of the 19 QMs listed on Nursing Home Compare. The nursing home’s clinical data serve as the basis for QMs.
The staffing rating is determined by two factors: the total number of staffing hours per resident day and the number of registered nurse (RN) hours per resident day. RNs, licensed practical nurses or licensed vocational nurses, and certified nurse aids make up the total staffing. The facility submits staffing data that are adjusted to meet the requirements of the residents.
