Download NSFAS Consent Form 2024-2025: NSFAS requires sensitive information about the employment status and income of applicants and their guardians from organizations. Therefore, parents or guardians of NSFAS applicants are provided with a consent form to sign as evidence that they have provided NSFAS with their personal information. This form can be obtained in PDF format for the 2024-2025 academic year from the NSFAS website, www.nsfas.org.za.

Documents required before signing the consent form for NSFAS 2024:

Provide your phone number and email address.
You must have a copy of your ID or birth certificate.
Copies of the national identification cards of parents, guardians, or spouses.
If necessary, provide proof of income or one year’s worth of IRP5.
Proof of income for your parents, guardians, or spouse.
If you are a SASSA grant recipient, there is no need to provide proof of income.
All applicants with a disability should submit a completed and signed Disability Annexure A Form.
An applicant recognized as a Vulnerable Child by the Department of Social Development must also submit a completed and signed Vulnerable Child Declaration and Consent Form.

Where To Submit Nsfas Consent Form

After you finished filling out the application form and the consent form of Nsfas, scan or email your completed application form and supporting documents to NSFAS at apply@nsfas.org.za.
